110-300-0215(3)(a)(i): The EpiPen medication was missing a label with the child's first and last name, the date the prescription was filled, and the name of the prescribing healthcare professional. 110-300-0215(3)(a)(ii): Benadryl was observed labeled with the child's name but was not accompanied by a medication authorization form. Since the medication requires a doctor’s authorization for administration, this absence of documentation is a compliance issue.
